If you are prescribing Clobetasol to any of your patients, please make sure to submit a preauthorization request form to our pharmacy department first. We now require preauthorization for this corticosteroid. Refer to these pharmacy preauthorization criteria for more information. This only applies to ConnectiCare's commercial plans.
