Registration will be available on or after January 5, 2023.
If you haven’t already, schedule your annual visit with your primary health care professional! This is a great way to start taking care of your health and to start earning rewards.
Once you register for the program and earn your first reward, a reloadable reward program card will arrive in about eight to ten weeks after your first health service or activity, and possibly sooner, depending on when we complete the claim for the eligible health service from your health professional. Your card will continue to reload with each completed eligible service or activity. You can use your reward card to pay for eligible products such as healthy foods, health necessities, and transportation services. Remember to keep your card and present it when you are redeeming rewards.
To see specific services and reward dollar amounts after you register, sign in to your member portal. Purchase and merchant restrictions apply. Once you register for the program, you will see which services could earn you rewards. In order to process your reloadable reward card for an eligible health service, you must register for the program, complete the health service, and the claim or attestation (available 10/1/23 to 12/31/23) must be received by Dec. 31, 2023. All activities for rewards and reward redemption cannot be extended beyond Dec. 31, 2023. Only one reward can be earned for each health service. Your reloadable reward card cannot be exchanged for cash, and cannot be used to purchase firearms, liquor, or tobacco. More details are available online.
