Your ConnectiCare patients with commercial and Medicare Advantage plans effective on or after Jan. 1, 2017, will have new member ID cards. We will also be issuing new member ID numbers to all of your ConnectiCare Medicare Advantage patients.

The redesigned member ID cards will make it easier for your office to identify the different product names, copayments and where you should send your claims for payment. It will also clearly note whether referrals are needed for your patient.

Here are some samples of the new commercial and Medicare ID cards.

Information for your front office:

  • All ConnectiCare Medicare members will have new member ID numbers and pharmacy information. Please make sure to use the new member ID when submitting Medicare claims for services after Jan. 1, 2017.
  • You will still see commercial members with old ConnectiCare ID cards. New cards are being issued as commercial members have plan changes.
  • ID cards for ConnectiCare plans purchased on Access Health CT (Connecticut exchange) will no longer have a red banner across the top. They will look similar to the other commercial ID cards but will reference ConnectiCare Benefits, Inc. (see below), the ConnectiCare subsidiary that sells plans on the Connecticut exchange.
