Get Rewards for Making the Right Health Care Choice
With the ConnectiCare Medicare Member Rewards Program, you can receive a reloadable rewards card for eligible services like preventive care screenings. The rewards period begins January 1 each year.
If you haven’t already, schedule your annual visit with your primary care provider (PCP). This is a great way to start taking care of your health.
How Can I Join the Medicare Member Rewards Program?
NOTE: Registration for the program is required for you to earn your rewards.

Call ConnectiCare Medicare Connect Concierge at 800-224-2273 (TTY: 711). From Oct. 1 to March 31, you can call us from 8 a.m. to 8 p.m., seven days a week. From April 1 to Sept. 30, you can call us from 8 a.m. to 8 p.m., Monday through Saturday for help getting started
Once you agree to terms and conditions and join the program, you’ll see what services and activities can earn you rewards. Then, talk to your doctor about scheduling the care that’s right for you.
*If you choose to register for the ConnectiCare Medicare Member Rewards Program over the phone, you must agree to the program’s terms and conditions during your call. To get a copy mailed to you, call 800-224-2273 (TTY: 711). From Oct. 1 to March 31, you can call us from 8 a.m. to 8 p.m., seven days a week.
