ConnectiCare Medicare Member Rewards Program

With the ConnectiCare Medicare Member Rewards Program, you can receive a reloadable rewards card for eligible services like preventive care screenings. The rewards period begins January 1 each year.
If you haven’t already, schedule your annual visit with your primary care provider (PCP). This is a great way to start taking care of your health.
How Can I Join the Medicare Member Rewards Program?
NOTE: Registration for the program is required for you to earn your rewards.

Once you agree to terms and conditions and join the program, you’ll see what services and activities can earn you rewards. Then, talk to your doctor about scheduling the care that’s right for you.
*If you choose to register for the ConnectiCare Medicare Member Rewards Program over the phone, you must agree to the program’s terms and conditions during your call. To get a copy mailed to you, call 800-224-2273 (TTY: 711). From Oct. 1 to March 31, you can call us from 8 a.m. to 8 p.m., seven days a week. From April 1 to Sept. 30, you can call us from 8 a.m. to 8 p.m., Monday through Saturday.

Once you receive your reward card, please follow the steps below to activate it:
1. Call NationsBenefits at 877-239-2942 (TTY: 711) (Monday to Friday, 8 a.m. to 8 p.m.) or
2. Create an account and a password by visiting connecticare.nationsbenefits.com.
- Select the register button.
- Enter your member ID to continue.
- Then enter your name, member ID number, and date of birth.
- Choose your own personal password to use.
- Choose how to receive a password verification, either by email or text.
- Complete the process by entering the verification code sent to you.

- Sign in to the member portal to see your specific services, review reward dollar amounts, and receive your reloadable rewards card.**
- Please allow eight to twelve weeks after ConnectiCare completes a claim for an eligible health service for your rewards card to arrive in the mail.
- Remember to keep your reloadable card after each use. Please do not throw your card away. It will automatically reload each time you complete an eligible service or activity.
To process your rewards card for an eligible health service, you must register for the program and complete the health service or activity. The claim or attestation must be received by Dec. 31 of the calendar year. We will not be able to send you rewards for 2024 completed activities after Dec. 31, 2025. The Centers for Medicare and Medicaid Services (CMS) requires that all eligible members who opt-in to participate and earn rewards in a Medicare Advantage rewards program must redeem all rewards no later than December 31 of the current year and therefore any unredeemed rewards earned in 2025 will not be eligible for redemption in 2026. Only one reward can be earned for each health service. Your rewards card cannot be used at ATMs; for cash access or to make recurring payments; or for the purchase of gift cards, alcohol, tobacco, or firearms. Purchase and merchant restrictions apply.
