With your ConnectiCare plan, many preventive services are covered at no cost to you -- no copayments, no coinsurance, no deductibles. The preventive care services must be provided by a participating provider and are subject to age and gender requirements and limitation rules. Please refer to your membership agreement in your plan documents for a list of covered services under your specific plan.
