***Limitations and exclusions may apply

We offer options for formulary and plan designs that encourage people to take their medicines as directed while identifying potential threats like drug interactions or misuse of opioids. Members can get free shipping and 90-day refills on drugs they take to stay well through Express Scripts, our pharmacy benefit manager.

We work directly with doctors and hospitals throughout the state to help members get the right care at the right time in the right place. These relationships drive better preventive care, lower use of the emergency room, and fewer hospital days.*

*ConnectiCare internal analysis of member and provider collaborations vs. members not in provider collaborations for 2019

Our nurse care managers, social workers, pharmacists, health coaches, and navigators reach out to your employees and covered dependents with chronic conditions (or who appear to be at risk). Those conditions include diabetes, heart disease, respiratory illnesses, tobacco dependence, high-risk pregnancies, autoimmune diseases, and others. Through education, coaching, and coordinating with providers, they help see that members get the care they need, take medicines as prescribed, and receive appropriate psychological, social, and emotional support.

Our award-winning customer service is here when you need us, over the phone or in person.** Connecticut-based representatives answer calls about coverage and connect members to resources in their communities. Members can visit ConnectiCare centers for face-to-face help as well as a variety of community events, including exercise classes, seminars, and more.

**The Stevie Awards for Sales & Customer Service is an awards program produced by the Stevie Awards®. ConnectiCare’s worksite wellness programs, offered through our affiliate company, WellSpark Health, focus on identifying and lowering employees’ modifiable risks. These programs, based on a foundation of research and experience, help engage employees in healthy activities and promote lifestyle changes to prevent chronic disease.
