Special Needs Plan Model of Care Provider Training
ConnectiCare is required to conduct training for providers regarding our Model of Care (MOC) program for Special Needs Plan (SNP) enrollees. The MOC is the foundation for ConnectiCare’s care management policy, procedures, and operational systems for our SNP population.
To ensure that ConnectiCare remains compliant with Centers for Medicare and Medicaid (CMS) regulatory requirements for MOC training, ConnectiCare Choice Network providers must take the Molina MOC training, and submit the applicable Attestation below either upon joining the network, or no later than Dec. 31, 2028.
If you are an individual provider, click Individual Attestation. If you are attesting on behalf of a group, click Group Attestation. A group is more than one provider who shares a valid tax identification number (TIN). You must enter a valid group TIN for the providers in the group to receive credit for completing the training. A separate attestation is required for each group/organization TIN
