Beginning July 1, 2026, elective diagnostic and interventional cardiovascular services rendered in a physician’s office, ambulatory setting, outpatient hospital, or *inpatient hospital setting (*professional services only) will require authorization from Evolent. This authorization requirement applies to all provider specialties for ConnectiCare Medicare members 18 years of age and older. Treatment plans will be reviewed using nationally recognized evidence-based guidelines.
Prior authorization requests for services scheduled on or after July 1, 2026 must be sent to Evolent online or by phone:
- Sign in to the Evolent provider portal.
- Call 888-999-7713, Option 1 – Cardiology, from 8 a.m. to 8 p.m., Monday through Friday.
