ConnectiCare has made updates to two prior authorization lists for our commercial (employer group) members. See the revision histories for changes and their effective dates for the following lists:
- ConnectiCare Pharmacy Preauthorization List
- ConnectiCare Prior Authorization Requirements (Commercial)
Note: the updates to these prior authorization lists apply to changes for 2025 dates of service. These are newly FDA-approved/new to market drugs. Revision history also indicates that these prior authorization lists do not apply to ConnectiCare Marketplace (on and off-Exchange) or Medicare plans for dates of service on/after Jan. 1, 2026.
