Exclusively for ConnectiCare Members

ConnectiCare members (and their caregivers) can get personalized help from nurse care managers, a certified diabetes educator, or a social worker at our center in Manchester. They can help with:

  • Learning to use a glucometer
  • Healthy eating for individuals with diabetes
  • Finding senior meal programs, elder care, social activities, and support groups
  • Guiding members to community resources
  • Managing a chronic condition
  • Learning about your prescription medicines
  • Learning to use a blood pressure cuff

There’s no charge to members. It’s all part of what we do to try to keep our members healthy.

Service available at the ConnectiCare center in Manchester. You must make an appointment. Call us at 1-877-523-6837 (TTY: 711). We will help you find the right professional to meet with and book your appointment.

Can’t meet with a care manager in person? You can also call to talk to a health care professional or health navigator. Call 1-800-390-3522 (TTY: 711) Monday - Friday, 8 a.m. to 4 p.m.


Please note that these health care professionals do not and will not provide medical care. For medical care, please make an appointment with your primary care doctor or visit an urgent care center.
