Health plans are required to mail Massachusetts residents a 1099-HC form, which serves as proof of coverage, by January 31 every year. ConnectiCare mailed a 1099-HC on January 27, 2009 to all subscribers who had a Massachusetts address and were enrolled at any time during the year ended December 31, 2008. On demand printing will also be available for any ConnectiCare subscriber requesting a 1099-HC form.
