The HeartCare Program

Having heart disease can make everyday activities difficult. HeartCare
includes two programs that address the varied needs of our members
with heart problems.

  • HeartCare – Heart Failure Program – provides support in coping with this chronic disease, as well as helping you recognize the symptoms of worsening heart failure.
  • HeartCare – Coronary Artery Disease (CAD) Program – provides education and support when you must make necessary lifestyle modifications to lower your risk of cardiac events.

With both of these programs, you'll have a Nurse Case Manager who can help you:

  • Set goals for a healthy lifestyle
  • Understand how to take your medicines and why your doctor prescribed them
  • Understand how other health conditions can impact your heart health, such as diabetes, high blood pressure, depression, and breathing problems
  • Know what to discuss with your doctor
