The DiabetiCare Program

Careful management of diabetes can greatly reduce the possibility of
complications. As a DiabetiCare participant, you benefit from educational
mailings, classes on diabetes and the opportunity to test your blood sugar
levels with a glucose monitor at home.

A Nurse Case Manager is available to help you stay in control of your diabetes by:

  • Keeping you up to date on the latest American Diabetes Association goals and guidelines
  • Reminding you when important tests are due – like the Hemoglobin A1c – and encouraging you to know the results
