ConnectiCare offers the following programs to support members and assist them with the coordination of services to address their special health care needs.
Special Care Case Management Program
If you are dealing with multiple health conditions or a serious health problem,
we know that you will need special attention. ConnectiCare's Special Care
Case Management Program is designed to focus on chronic illnesses and
their management challenges.
You will be assigned a Nurse Case Manager or Social Worker Case Manager
who will provide you with guidance and support for:
- Medical care and how to best access it
- Community Resources
- Self care
- Medication compliance
- Lifestyle changes
- Equipment and supplies you may need

Transplant Case Management Program
This program is designed to provide ConnectiCare members and their families with education, support and advocacy. A registered nurse with experience in transplant management calls members to help them obtain and coordinate necessary health care services, while helping them maximize health care benefits. The nurses are a resource regarding transplants, cost issues, community resources, and care options.

Specialty Pharmacy Management Programs
ConnectiCare contracts with pharmacies that specialize in prescription
drug therapies that require unique handling for the following conditions:
- Hemophilia
- Hepatitis C
- Multiple Sclerosis
- Infertility
By doing so we are able to provide our members, who use these
products, a level of service not possible from a local retail pharmacy.
In addition, these pharmacies offer 24/7 nurse and pharmacist
consultation for side effect, administration and storage questions.
