Cancer Support Program
ConnectiCare, through an arrangement with OptumHealth, offers a variety of
resources to encourage better patient participation and compliance with the
therapeutic care plan you have established. People with cancer can often benefit
from personal, telephonic outreach from an oncology nurse who can answer
questions and help the member navigate the health care system. The Cancer
Support Program complements your treatment plan and reinforces the instructions
you have provided.
ConnectiCare commercial members who have been diagnosed with cancer and are in
active treatment or end-stage management can participate in the program. Member
participation in the program is voluntary. Once enrolled in the program,
patients are assigned to an oncology-trained, registered nurse who will provide
short-term assistance or ongoing case management. For people who receive
on-going case management, the Cancer Nurse Advocate will offer individualized
support to the member and family caregivers and help the member during treatment
and recovery or end-of-life care.
