Member Disenrollment
• ConnectiCare reserves the right to terminate coverage for members who
repeatedly fail to make the required copayments, coinsurance or deductibles,
subject to the terms outlined in the applicable Member Agreement, Evidence of
Coverage, or other governing contract.
• Providers are responsible for seeking reimbursement from members who have
terminated if the services provided occurred after the member's termination
date.
