How do I obtain a Large Group Quote?
In order to obtain a quote for a CT or MA domiciled employer group with 51+ eligible employees, send the following information to:

- Census of all employees including Waivers. For each employee, specify date of birth, gender, family tier, and residence zip code.
- Evidence of insurance for the past two years.
- Current carrier's current rates and effective date.
- Current carrier's benefit description.
- Number of employees currently insured under the employer's group policy by carrier.
- Projected renewal rate and effective date.
- Other HMO involvement, with current and proposed rates.
- Employer contribution by subscriber category -- current and proposed.
- Number of health insured and not insured employees by subscriber category, also by age/sex category and by carrier.
- Historical premiums and claims experience for the last two to three years.
