      Free Blood Sugar Testing Systems

      ConnectiCare is pleased to offer the latest blood sugar testing systems—the ACCU-CHEK® and FreeStyle® meters--free of charge to members.

      To order an ACCU-CHEK® meter, call 1-888-355-4242.

      To order the FreeStyle meter, call 1-866-224-8892.

      Please have your ConnectiCare member ID number ready when you call. You should receive your new meter within 3-5 business days.

      ACCU-CHEK® and FreeStyle strips and lancets are covered under the ConnectiCare pharmacy benefit at the second-tier co-pay with your doctor’s prescription.
