Below are 5 ways to enroll in a ConnectiCare VIP Medicare Plan:
1. Enroll Online
To enroll in a ConnectiCare VIP Medicare Plan, click the Plan you wish to enroll in below. By completing the online enrollment process you will be submitting and actual enrollment request to join a ConnectiCare VIP Medicare plan.

ConnectiCare VIP Prime 4 (HMO)
ConnectiCare VIP Option 1 (HMO-POS)

Medicare beneficiaries may also enroll in a ConnectiCare VIP Medicare Plan online through the Centers for Medicare & Medicaid Services Online Enrollment Center by clicking here.

A ConnectiCare Representative will meet with you at a time and place that's convenient for you to answer your questions and assist you with completing the enrollment application.
