How to Enroll for a ConnectiCare VIP Medicare Plan

  Here's 4 ways to enroll in a ConnectiCare VIP Medicare Plan:

  1.Call us toll-free at 1-877-224-8221 (or for TTY/TDD users: 1-800-842-9710) to:

  • Schedule a personal consultation with a ConnectiCare Representative who will answer your questions and assist you with completing the enrollment application.
  • Reserve a seat for you at one of our informational seminars in your area, where you can find out more about our ConnectiCare VIP Medicare Plans and a ConnectiCare Representative will be present and can assist you with completing your enrollment application. For accomodations of persons with special needs, contact us at the toll-free number listed above.
  • Request an enrollment kit.

  2.Enroll Online.   Medicare beneficiaries may enroll in a ConnectiCare VIP Medicare plan through the Centers for Medicare & Medicaid Services Online Enrollment Center.
   

  Enroll Online Medicare beneficiaries may enroll in a ConnectiCare VIP Medicare plan through the Centers for Medicare & Medicaid Services Online Enrollment Center starting November 15, of the current year. Choose your Plan: ConnectiCare VIP Prime 1 ConnectiCare VIP Prime 2 ConnectiCare VIP Prime 3 ConnectiCare VIP Prime 4 ConnectiCare VIP Custom 1 ConnectiCare VIP Option 1 ConnectiCare VIP Option 2

  
  3. Enroll by Phone. Medicare beneficiaries may enroll in a ConnectiCare VIP plan over the phone by calling us toll free at 1-877-224-8221 (or for TTY/TDD users: 1-800-842-9710).

  
  4.Enroll by mail. Medicare beneficiaries may enroll in a ConnectiCare VIP plan by completing an enrollment application. If you have any questions a ConnectiCare Representative is available to meet with you who will answer your questions and assist you with completing the enrollment application. Call us toll-free at 1-877-224-8221 (or for TTY/TDD users: 1-800-842-9710) to schedule an appointment.
  

  Typically, you may enroll in a Medicare Advantage plan during the annual enrollment period between November 15 and December 31 of each year. In addition, you can join a Medicare Advantage plan during the open enrollment period between January 1 and March 31 of each year, as long as you do not change your prescription drug coverage. Additionally, there are exceptions that may allow you to enroll in a Medicare plan outside of these periods. Please complete the Attestation of Eligibility for an Enrollment Period form to see if you are eligible for an Enrollment Period.
  
  ConnectiCare offers a Direct Debit program making it easier to pay your monthly premium payment. Each month ConnectiCare deducts your premium directly from your bank account. There are no monthly premium invoices and no premium checks to write. The Direct Debit program saves you time, paperwork and the worry that you will forget to pay your premium. If you would like to sign up for the Direct Debit program, please complete the Direct Debit Program Application Form and mail it in with your enrollment application.

  
  
  If you are already enrolled in a ConnectiCare VIP Medicare Plan and have questions, contact us toll-free at 1-800-CCI-CARE (1-800-224-2273). For the hearing and speech impaired, call 1-800-842-9710 (TTY/TDD). You can call either number from 8:00 a.m. to 8:00 p.m., seven days a week.
