SilverSneakers gives you a fitness membership with access to more than 11,000 fitness locations across the country, with all basic amenities and SilverSneakers fitness classes designed specifically for active older adults.
- In your favorite neighborhood locations
SilverSneakers FLEX™ brings classes such as tai chi, yoga, dance and walking groups to parks, recreation centers, and other local places where you already go. You can take FLEX classes and still participate in SilverSneakers at fitness locations.
- At home or on the go
If you can’t get to a fitness location, sign up for SilverSneakers Steps®, a personalized fitness program that lets you choose one of four kits that best fits your lifestyle and fitness level – general fitness, strength, walking or yoga.
At the SilverSneakers website you can use tools to assess your health and track your activity, find fitness advice, download healthy recipes and interact with the SilverSneakers community.
Health & Wellness Support Services
As a ConnectiCare member, you’ll have access to many health and wellness resources and services. These are offered at no additional cost beyond the monthly Plan premium.
Important Health Reminders
Staying on top of your health is important. That’s why ConnectiCare provides information mailings, phone calls and postcards to encourage the use of needed preventive health services, screenings and immunizations. These may include:
- Cholesterol Screening
- Colorectal Screening
- Osteoporosis Screening
Medication Therapy Management Program – This program will help you understand and manage your medications. A pharmacist or health care provider will review your prescription medicines, over-the-counter medicine and any herbal supplements you may take. The goal is to make sure they are safe and working to improve your health.
Diabetic Eye Exam Reminder – An annual eye exam is very important for members who have diabetes. That’s why we may send a friendly reminder when an eye exam is due or overdue.
Treatment Decision Support Program
If you have health problems that require treatment, you may have questions or need someone to talk to about medical decisions. Our Treatment Decision Support Program will help you make confident decisions about your personal situation. In this program:
- Members can speak directly with a specially trained Registered Nurse
- The Nurse will answer your questions and give objective, relevant health care information
- Registered nurses cannot diagnose problems or recommend specific treatment. However, they can help you work more effectively with your doctors
Medicare Care Management Program
Complex health care needs require special attention. This program focuses on supporting our members with chronic or complex medical needs.
We identify members for this program through a Health Assessment, a post-discharge follow-up call, or based on a review of claims information. Once identified, a Registered Nurse Care Manager will work with you to:
- Identify any gaps or concerns you may have
- Help understand your goals of care
- Connect you to a care management team to help you meet your goals
