ConnectiCare Touchpoints Helps Members Manage Their Condition: New Cancer Support Program Added

At ConnectiCare, our members’ health is at the heart of everything we do. That’s why we’re pleased to now offer ConnectiCare Touchpoints, our comprehensive program of resources, services and information designed to empower members to make informed health care decisions, and offer them medical and support services when they need them.

 

New Cancer Support Program Added To ConnectiCare Touchpoints

From preventive health reminders to hospital and doctor guidance, Touchpoints has ways to help members manage their condition and stay in control.

Our Care Management Programs are an important part of Touchpoints. These programs help members manage their special health care needs. Our newest addition is the Cancer Support Program.

Cancer Support Program

If someone is diagnosed with cancer, they often have questions and need to make difficult decisions. Our Cancer Support Program provides members with a single source of personal support from an experienced cancer nurse. The nurse will work with members and their loved ones as long as they have needs regarding cancer. The program addresses all kinds of cancer, and is available at no additional cost to members’ health benefits.

Besides helping our members cope with the emotional side of cancer, the nurse can provide information about treatment options, answer questions, or guide members to a provider in their local community or within the OptumHealth Cancer Centers of Excellence network. (OptumHealth, Inc. performs cancer-support services on behalf of ConnectiCare, Inc. & Affiliates.)

The Cancer Centers of Excellence are nationally respected organizations chosen because of their quality results. Each center features:

  • Specialized teams of clinicians with extensive experience in cancer diagnosis and treatment, including complex and rare cancers;
  • Second-opinion services to confirm a diagnosis or recommended treatment;
  • Experience performing a large number of cancer surgeries and other complex treatments and procedures.

Each person with cancer has different needs. Members are encouraged to receive care in consultation with their physician, based on their personal values, needs and preferences. For more information, members may call toll-free 1-866-897-1038.

Members are not obligated to use services of the Cancer Support Program or receive treatment at the Cancer Centers of Excellence. Use of the Cancer Centers of Excellence is not guaranteed. Treatment options and practitioner expertise within the ConnectiCare participating provider network will be reviewed prior to using the Cancer Centers of Excellence.

Special Care Case Management Program: If members are dealing with multiple conditions or a serious health problem, we know that they will need special attention. This program is designed to focus on chronic illnesses and their management challenges.

Transplant Case Management Program: Designed to provide ConnectiCare members and their families with education, support and advocacy throughout the transplant process.

Kidney Case Management Program: Designed to help ConnectiCare members and their families manage kidney disease. A registered nurse with specialized training calls members to provide guidance and support -- and to monitor health conditions and complications that are commonly related to kidney disease.

Health Management Programs: for Specific Conditions

Touchpoints also has comprehensive Health Management Programs to help members manage specific conditions. For information on the following programs, members may call toll-free 1-800-390-3522:

  • BREATHE – Asthma: for all members with asthma.
  • BREATHE – COPD: for members with chronic obstructive pulmonary disease (COPD), which includes chronic bronchitis and emphysema.
  • DiabetiCare – for adult members with diabetes.
  • HeartCare – CAD: for members with coronary artery disease.
  • HeartCare – HF: for members with heart failure.
  • Birth Expectations – for pregnant members with a history of a previous preterm delivery or of carrying more than one baby.
