| Confirmed with Alternative Plan | This status indicates that the ConnectiCare suggested alternative plan design has been selected and confirmed as the renewal plan design for this division. This plan and its rates will become effective on the group's renewal date. |
| Confirmed with New Plan Option | This status indicates that additional division(s) have been added to an existing group. This status is displayed when a group adds a dual or triple option. |
| Confirmed without Changes | This status indicates that the division has been renewed with the current plan design and renewal rates. This plan and its rates will become effective on the group's renewal date. |
| Confirmed with Requote | This status indicates that a plan design option quoted by a user has been selected as the renewal plan design for this division. This plan and its rates will become effective on the group's renewal effective date. |
| Default Renewal | This status indicates that a response was not made for this division by the renewal response date (one month prior to the employer group's effective date). The division has renewed with the current plan design and renewal rates. |
| Division#/Name | A division is created for each plan design or category. A division # consists of a 6 digit group number and a 4 digit suffix (e.g. C12340001) |
| Do not Renew | This status indicates that a group has changed from multiple divisions to fewer divisions (e.g. dual to single option). If you wish to terminate coverage for a group, please submit written notification to ConnectiCare's Group and Membership Administration Department. |
| Do Not Renew Division | Select this action link to remove an option plan design that the employer no longer wants to offer (e.g. dual to single option). If you are replacing an existing plan design, please run a requote from an existing division. |
| Generate Initial Renewal Package | Select this link to generate a renewal package with the current plan and current rates, current plan and renewal rates and a ConnectiCare suggested plan alternative. The initial renewal package will reflect the most current census and group information submitted to ConnectiCare within the last 24-48 hours. You may print, view, email or fax this document |
| Generate Requote Package | Select this link to generate a customized proposal that may include the current plan with current rates, current plan with renewal rates, ConnectiCare suggested plan alternative and requote options. |
| Group ID# | The number assigned to a group by ConnectiCare. The group ID# is 6 digits in length (e.g. C12345). Additional plan designs and categories are identified as divisions within a group. These can be identified by an additional 4 digits at the end of a group number (e.g. C12340001). |
| Group Name | The name of the employer group as submitted on the small employer group application. |
| Group Size | The rating category based on the total number of full time eligible employees. Current categories for small group are 1-2, 3-9, ad 10-50) |
| Group State | The state in which the employer group headquarters is domiciled. |
| Monthly Premium | On the Search Results page, the monthly premium refers to the current plan and renewal monthly premium for the specified division. |
| Not Available for online processing | The renewal is not available for online processing by external users. Please contact Small Group Sales at 1-800-723-2986. |
| Plan Name | The name of the small group plan design. A summary of benefits can be viewed via the Plan Summary Page |
| Producer Name | The name of the Agent of Record provided by the employer group. |
| Quote Alternatives | Select this action link to perform a requote of a new plan design using the existing census for this division. |
| Renewal Effective Date | The date the rate and plan design changes will be effective for the employer group. |
| Requote# | The number assigned by the renewal application to track requote options performed by users. |
| Saved Requote | This status indicates that an optional plan design has been quoted and saved for consideration by the group. |

| Terminated | This status indicates that the group is no longer active with ConnectiCare. Please contact Group and Membership Administration at 1-800-333-1733 |
| Total Eligibles | The total number of full time eligible employees as defined by ConnectiCare's underwriting guidelines. The total eligibles is used to determine the group size category. |
| View Census | Select this action link to view a list of current subscribers. Name, Date of Birth, zip code, status, gender, and coverage level are also displayed. |
| View/Renew | Select this action link to view the division's current plan design with current and renewal rates as well as a ConnectiCare suggested alternative plan design. This link allows you to perform a requote option and to renew a division online. |
