ConnectiCare's prescription drug program uses a 3-tier copayment structure to provide you with access to nearly all prescription drugs on the market and also help you control what you spend on prescription drugs. Please check your specific membership materials to determine your exact prescription copayment amounts.

 

The 3-tier prescription drug program divides medications into three categories.

 
Tier Description Amount 1
Tier One Primarily Generic Drugs and some brands lowest copayment
Tier Two Certain Brand Name Drugs and some generics intermediate copayment
Tier Three Certain Brand Name Drugs and some generics highest drug copayment

1To determine your actual copayment amount, please refer to your Benefit Summary. Copayment amounts are per month supply. If the retail cost of the drug is less than the copayment, you pay the retail cost of the drug. In most plans, if you choose a brand name when a generic is available, an additional payment is required.
