Tools That Help our Member to Take Charge of Their Health

The Personal Health Manager is suite of online member tools provided by  that serve up customized information and support based on an individual’s reported interests, risks and readiness for change. These tools include:

• My Health Assessment – scores an individual’s health status, calculates a Health Risk score, and compares the results with others of the same age group and gender.
• Condition Centers® – provides in-depth assessments and information about specific health conditions, such as asthma, high cholesterol, depression, chronic fatigue, diabetes and much more.
• Personal Health Record – serves as a confidential place for individuals and their families to keep health records that they can print out and bring to doctor visits.
• Improvement Programs – guides users through positive lifestyle changes, including enhanced fitness, improved nutrition, healthy pregnancy and smoking cessation.
• Trackers & Reminders – helps users monitor their progress and chart activity—whether it’s controlling daily glucose levels, lowering blood pressure or exercising regularly.
• Child Health Manager – helps parents and guardians keep track of health care visits, immunizations, health conditions, and growth and development in children age 6 and younger.
• Health Topics A-Z – find out about all things health related, the symptoms, detection, treatment and prevention.
