Health Management Programs:
Member Rights and Responsibilities

You are encouraged to actively participate in decision making with regard to managing your health care. As a ConnectiCare member participating in Health Management Programs, you have a right to:

  • Have information about ConnectiCare's Programs and services including the staff, the staff qualifications and any contractual relationships related to Health Management Programs.
  • Decline participation or disenroll from health management programs and services offered by ConnectiCare.
  • Know which staff members are responsible for managing health management services for the member and from whom to request a change.
  • Be supported by ConnectiCare when making decisions interactively with their practitioners regarding health care.
  • Be informed of all health management-related treatment options included or mentioned in ConnectiCare clinical guidelines, whether covered or not by ConnectiCare and to discuss these with the treating practitioners.
  • Have personal identifiable data and medical information kept confidential; know what entities have access to member information; and know procedures used by ConnectiCare to ensure privacy and confidentiality.
  • Be treated courteously and respectfully by ConnectiCare staff.
  • Communicate complaints to ConnectiCare and receive instructions on how to use the complaint process, including knowing the organization’s standards of timeliness for responding to and resolving issues of quality and complaints.
  • Receive information that is understandable.
  • Have ConnectiCare act as a member advocate.

While enjoying specific rights of participation in Health Management Programs, you will also assume the following responsibilities. You will have a responsibility to:

  • Follow the care support and education offered by ConnectiCare.
  • Provide ConnectiCare with information necessary to carry out its services.
  • Notify ConnectiCare and the treating practitioner if the member decides to disenroll from the program.
