Health plans are required to mail Massachusetts residents a 1099-HC form, which serves as proof of coverage, by January 31, 2008. ConnectiCare will mail a 1099-HC by January 25, 2008 to all subscribers who have a Massachusetts address and are enrolled as of December 31, 2007. On demand printing will also be available for any ConnectiCare subscriber requesting a 1099-HC form.
